Interwell Health’s Value-Based Kidney Care Earns NCQA Accreditation

The National Committee for Quality Assurance (NCQA) awarded value-based kidney care provider Interwell Health a three-year year Population Health Program (PHP) Accreditation for Interwell 360. Interwell 360 is a comprehensive care management program for people living with chronic kidney disease (CKD).

The accreditation acknowledges strong performance in the functions outlined in NCQA’s standards for Population Health Program (PHP) Accreditation. NCQA accreditation standards are developed based on input from various stakeholders and resources including health plans, population health management industry leaders and organizations, an expert panel, and standing committees.

Interwell contracts with regional and national payers to deliver value-based care. It also partners with physicians to improve the health of their CKD patients while lowering the total cost of care. Interwell’s interdisciplinary care team includes nurses, dietitians, social workers, and care coordinators. Using predictive models, the company identifies patients most at risk of progression and hospitalizations, while a specialized care team conducts rapid outreach post-discharge to prevent readmissions.

Fresenius Health Partners achieved NCQA PHP Accreditation in January 2022 for its KidneyCare:365 program prior to becoming Interwell Health in a merger later that year. This is the company’s first NCQA review and audit since the merger.
