The funding will help Evergreen expand into new regions and implement advanced technologies to help improve clinical outcomes, streamline delivery of care, and lessen the administrative burden on nephrologists.

Evergreen offers nephrologist partners wraparound support through analytical insight and tools and a dedicated patient care team including nurse care managers, care coordinators, social workers, dietitians, and pharmacists. The goal is to ensure holistic and personalized patient care while allowing nephrologists to focus on their clinical expertise and leadership in kidney disease management.
