
Aetna, a CVS Health company, announced that it will offer in-home care to its Medicare Advantage members who have chronic kidney disease (CKD) and end-stage kidney disease. The effort is a partnership with Monogram Health, a provider of in-home care management services. Previously, CVS Health Ventures, the venture capital arm of CVS, made a strategic investment to support Monogram’s growth and expansion.
In a LinkedIn post, Monogram explained, “Eligible Aetna members will have access to Monogram’s comprehensive care model and in-home and virtual specialty provider appointments, led by Monogram nurse practitioners, under the direction of nephrologists and other physician specialists. Monogram and Aetna have also clinically aligned to encourage preemptive referrals for kidney transplant evaluation to deliver better long-term outcomes for patients.”
