There are “significant lung cancer mortality inequalities” in the United States, according to a new study.
Researchers led by Sabrina Soin, DO, of the University of Arizona, conducted the study and presented their findings during the American Thoracic Society 2024 International Conference.
Dr. Soin and colleagues evaluated lung cancer mortality rates with an “emphasis on yearly mortality shifts and disparities” because lung cancer, which is “one of the leading causes of morbidity and mortality” in the United States, is “often impacted by demographic and socioeconomic factors,” they explained.
The researchers obtained lung cancer mortality information spanning 1999 to 2020 from the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research database. The death certificate queries included the International Classification of Diseases, Tenth Revision codes of C34.0, C34.1, C34.2, C34.3, C34.8, C34.9. The researchers also obtained demographic information on age, sex, race/ethnicity, and the area of residence. They used this information to compare age-adjusted mortality rates (AAMR) per 100,000 population across demographic subpopulations. In addition, they used log-linear regression models to assess temporal mortality shifts and calculate the average annual percentage change (AAPC) in mortality.
The study identified 3,380,830 lung cancer deaths and found that the AAMR significantly decreased from 55.41 in 1999 to 31.84 in 2020 (P&lt;.001). However, the AAMR among males (57.63) and non-Hispanic populations (47.56) was higher than the AAMR reported in females (36.04) and Hispanic populations (19.08).
The researchers also found disparities between metropolitan areas and nonmetropolitan areas.
“Although nonmetropolitan regions were affected by higher cumulative mortality rates, the annual decrease in mortality among metropolitan regions (AAPC −2.8%; P&lt;0.001) was more prominent compared [with] nonmetropolitan regions (AAPC, −1.7%; P&lt;.001),” the study authors reported.
In addition, the Western US regions “encompassed the most prominently decelerating mortality shifts compared [with] the other US census regions,” showing an AAPC of −3.4% (P&lt;.001).
“Our findings revealed significant lung cancer mortality inequalities in the [United States], based on demographic factors,” Dr. Soin and colleagues concluded. “By contextualizing these mortality shifts in the previous 22 years, our goal is to provide a larger framework of data-driven initiatives for societal and health policy changes for improving access to care, minimizing health care inequalities, and improving outcomes.”
